Mrs. Willis is complaining of sharp pain rated as 8 out of 10 in her right chest exacerbated with respiration. She is alert, oriented to person and place, but not time and situation. She is cooperative at this time, though she is distressed due to pain and shortness of breath. She is slightly confused with certain standard questions and instructions. You auscultate no breath sounds on the right side and diminished breath sounds with some crackles on the left side.
