The risks and benefits of this procedure were discussed in detail. Specifically, Mr. ___ [NAME] is aware that his vision will still be limited by Salzman's nodular degeneration and also that there is some risk of corneal decompensation resulting from this procedure. He opted to proceed and was properly consented. It should be noted that during the preoperative workup, there was difficulty obtaining an ultrasound measurement of the AP diameter of the eye due to poor lens echos. Therefore, a second attempt was made to obtain these results in the preoperative holding area. A total of four readings were obtained which were all significantly higher than previous measurements performed at the [PLACE]. On the basis of these measurements and after discussion with Dr. ___ [NAME], it was decided that the planned lens power should be decreased to 30.0 diopters in a Chiron, model C11UB lens. Also, in the preoperative holding suite, a local retrobulbar anesthetic block was administered which consisted of lidocaine and bupivacaine. This was well-tolerated and provided effective anesthesia
