2- h(x)=√x+3 is horizontally translation to the left and not the right it has to be √x-3.

3- f(1)= 4*1 = 4  y1

 f(4)= 4*4 =16    y2

step 2 : f(4)-f(1)/4-1 = y2-y1/x2-x1 = 16-4/4-1=12/3=4

the average rate of f(x)=√4x is 4
