How could the skin cells under the victim’s fingernails indicate that a struggle occurred?

Skin cells under a victim's fingernails shows that they gave up a fight for their life and fought back against their attacker. Scratching the attacker will result in skin cells under the fingernails which proves a struggle occured. 

They probably don't understand either 1. what the nightmare is about or 2. his anxiety and stress
